How to Update Credit Card or eCheck Payment Information

Refund Policy

If player withdrawal is necessary, it is the member’s responsibility to immediately notify the Director of Hockey and follow up with a written request for a refund. The Director of Hockey and Treasurer will determine the appropriate refund.

Jerseys and Volunteer Obligation Deposits

Mini Mites (IP), Mites & House Rec: 

  • No volunteer hours or volunteer deposit required 
  • Mites & House Rec are required to give a $100 jersey deposit*

Squirts through Bantams:  

  • 20 hours per player (maximum of 40 hours per family), $300.00 volunteer deposit/player
  • ALL players are required  to give a $100 jersey deposit*

High School (Midgets):

  • 20 hours per player (maximum of 40 hours per family) $300.00 volunteer deposit per player
  • Midget/H.S. players will buy and keep their jerseys and socks, this cost is $200/player.

All GSYHA players are required to have game jerseys and socks, players are required to purchase their own socks from GSYHA (except first year travel players). Please contact your Director if you need to purchase game socks.

*Jerseys will not be issued until team managers have received the required deposit and signed volunteer agreement.

For best results, save the PDF to your computer.  Some of these forms can be filled out while open in Adobe Reader, then printed for signature. Depends on which browser you are using:  right click the link and select “Save Link as …” in Firefox, “Save Target As …” in IE, “Download Linked File as …” in Safari, or “Save Link as…” in Google Chrome, then save it to your hard drive and open from there in Adobe Reader.  If it's a form, fill it out in Adobe Reader, then print for signature.  If you can scan the signed form, please email to
